SFCC Report It

Use this page to report non-urgent situations involving student conduct, students of concern, and students needing academic support

IMPORTANT NOTE about situations requiring an immediate response:

  • Do not use these forms to report emergencies or situations requiring an immediate response.
  • For urgent medical, mental health, or safety concerns, call 911 or contact Campus Security through the CampusShield app (preferred) or 509-533-3333. If you call 911, please call Campus Security to alert them to the call.
  • Additional resources for mental health emergencies are 988, the National Suicide and Crisis Lifeline, and the 24/7 Regional Crisis Line at 1-877-266-1818.
  • A list of non-urgent mental health referral resources is provided below.

For student conduct, BIT/Cares, or Title IX situations that are not emergencies but require quicker attention, please submit an on-line report form and also call or email Dr. Rhondie Voorhees, Dean of Students (for student conduct and BIT/Cares cases) or Dr. Jim Mohr, Title IX Coordinator and Vice President of Student Affairs (for Title IX cases).

If you are unsure about which reporting link to use, don't worry; choose the option you think most closely fits the situation. We will reroute reports internally if needed. 

Student Conduct - General

For all behaviors listed in the Standards of Conduct for Students, not including Academic Dishonesty. These reports go to the Dean of Students Office.

Student Conduct Report

Academic Dishonesty

For Academic Dishonesty and other ethical violations as outlined in the Standards of Conduct for Students, including cheating, unauthorized use of AI, knowingly furnishing false information, and forgery or alteration of documents. These reports go to the Dean of Students Office. According to the Standards of Conduct, suspected violations may be handled by the faculty member and referred to the Dean of Students Office for record keeping. More information is provided on the reporting form.

Academic Dishonesty Report

BIT/Cares Team Report

For serious concerns about threats of harm to self or others, or concerns about a student’s mental health and/or physical well-being. These reports go to the SFCC BIT/Cares Team, short for "Behavioral Intervention Team," convened by the Dean of Students. The BIT/Cares report may also be used to report concerns about Basic Needs, including housing insecurity, food insecurity, and financial insecurity.

Reminder: For any situation requiring an immediate response, call 911 or contact Campus Security.

Title IX Report, Student Cases

For sexual and gender-based discrimination, sexual harassment, sexual violence, domestic violence, dating violence, and stalking cases involving students. These reports go to the SFCC Title IX Coordinator who is also the Vice President of Student Affairs.

Hazing Report

For any suspected incident of hazing. More information coming soon. If you are concerned about a possible hazing incident, please contact the Dean of Students Office.

 

Academic Referral

Research shows that when students are struggling academically, the most effective intervention is for the instructor to reach out to the student to offer support or a referral. Faculty members and other employees may use this form to seek additional help for students with the following academic needs:

  1. Attending class but struggling
  2. Attendance concerns
  3. Math coaching
  4. Other academic coaching (not math)
  5. Assistance with technology
  6. Career services and planning

These referrals go to staff in Connected Learning and the Career & Workforce Development Center.
